David Moyes' side currently top Group A but face a nervy finale to their campaign, after two draws in their four games leave them just three points ahead of third-place Shakhtar.
United lead their German counterparts by just one point and a win for Sami Hyypia's side would see them leapfrog the Red Devils into first place and book Leverkusen's spot in the last 16.
If United fail to win, a victory for Shakhtar against bottom-side Real Sociedad will set up a Matchday Six shoot-out to determine who qualifies.
When the two sides met in September, a Wayne Rooney double helped the Red Devils to a 4-2 victory, with the England striker reaching a landmark 200 goals for United.
Moyes' team go into their penultimate group fixture as the form side in the Premier League with 14 points in their last six games, despite Sunday's disappointing 2-2 result in Cardiff.
Hyypia's side are in decent form too thanks to a three-match unbeaten run, including their 1-0 victory at Hertha Berlin on Saturday.
United have never lost to Leverkusen in the Champions League since their first encounter in 2002, and Moyes will be hoping to continue that with a qualification-sealing victory at the BayArena.
